Benefits Overview
HEALTH INSURANCE:
No cost to full-time employees. Family policies are available at extra cost. Part-time employees pay 45% of premiums. Tax free deduction for health insurance premiums. Must work 35 hours per week to be considered full-time. Must work 20 hours per week. Coverage starts one month after hire.

FLEXIBLE SPENDING:
Tax free dollars for un-reimbursed childcare and health care expenses. Can join upon hire or start of plan year. Must be scheduled 20 hours per week.

VOLUNTARY DENTAL, VISION AND CANCER INSURANCE PLANS
Employee’s may choose to purchase dental, vision, and/or cancer insurance coverage.

LIFE INSURANCE & LONG TERM DISABILITY:
Free to full-time employees. $5,000 life insurance, 65% long-term disability. Work at least 35 hours per week, and 90 day waiting period.

PENSION PLAN:
Variable Fund Pension Matches employee contributions of 1%, 2%, or 3% of employee’s wages. Must work for one year, have 1,000 hours, and be at least 19 years old.

GROUP SAVINGS PLAN:
Great way to save and earn higher interest. Must contribute $15.00 per pay period.

TAX DEFERRED ANNUITY:
Fixed and variable accounts. Minimum contribution of $10.00 per month.

PERSONAL TIME OFF:
Hours accrued for vacation, holidays and 3 personal days to total 168 hours the first year for full-time employees. Accrual rates increase after 3 years. Accrual for part-time employees will be proportionate to the number of hours worked per pay period. Personal Time Off cash out option available - special conditions apply.

EXTENDED ILLNESS BANK:
Full-time employees accrue extended illness hours at the rate of 9 days per year. Accrual for part-time employees will be proportionate to the number of hours worked per pay period.

EAP:
The employee assistance program (EAP) provides short-term, confidential counseling for you and your family at not out-of-pocket expense to you. APS Healthcare provides counseling services in collaboration with your employer or health insurer.

MIDRIVERS DISCOUNT:
All Sidney Health Center employees will receive a 10% discount, just on Internet services, through Mid-Rivers.

HEALTHWORKS MEMBERSHIPS:
Membership incentives are available to employees who sign up for memberships. If an employee signs up Sidney Health Center will pay up to $100.00 per individual employee that renews their membership or starts a new one. The employee is required to attend HealthWorks at least 8 times a month in order to receive this benefit.
