Extended Care Rehab Short Stay

Sidney Health Center Extended Care is offering a transitional rehab program. This transitional care is available to help patients safely recover after a hospital stay. Patients will receive skilled nursing care in addition to rehabilitation services to promote quality of life and continued prior levels of function until patients are ready to transition home.
